When your lower eyelid turns outwards, it can leave the delicate inner surface exposed and cause significant discomfort. This condition, known as ectropion, often leads to persistent redness, watering, and a feeling of grittiness or dryness. For many, this is a natural consequence of the tissues softening as we age, though it can also result from scar tissue pulling the eyelid away from the eye.

Eyelid Tightening
If your primary concern is an eyelid that has become loose with age and turned outwards, the most common approach is to gently tighten the lower lid. This usually involves a precise, small incision of around 10 to 15 millimetres placed at the outer corner of your eye. By delicately shortening and reattaching the eyelid to the underlying structure, we can encourage it to sit correctly against the eye surface once again. This process is typically completed in 30 to 60 minutes and aims to provide swift relief from irritation.
Skin Grafting for Sun Damage or Scarring
If the outward turning is caused by tight skin, perhaps from previous scarring or significant sun damage, we may need to adjust the approach. In these instances, we can borrow a very small patch of skin, usually taken discreetly from behind the ear or the upper eyelid, to provide the necessary support and flexibility to the lower lid.
Healing and Marks
We always aim to place incisions within the natural creases of your eye area to keep any marks as discreet as possible. In the initial weeks, the small incision line might appear slightly red or thickened, but this usually softens and becomes incredibly faint after a few months.
The early recovery period is quite manageable. You might experience some swelling, bruising, and a feeling of tightness for the first couple of weeks. We recommend sleeping with an extra pillow to keep your head raised, which actively helps reduce this swelling. We will also provide lubricating drops to keep your eye completely comfortable while it heals.
Non-Surgical Comfort Measures
While an operation is usually necessary to permanently correct the physical position of the eyelid, we can certainly use conservative methods to improve your comfort in the short term.
Using regular lubricating eye drops or applying an emollient cream can help protect the exposed surface of your eye from drying out. In some cases, gentle eyelid massage or wearing soft contact lenses can also act as a protective barrier. However, these remain temporary solutions, and surgically correcting the lid laxity is the most reliable way to prevent long-term complications like severe infections or damage to the cornea.
The best way forward is simply to have an open conversation. Every patient is completely unique, and the right choice depends on your specific symptoms, the health of your eye, and most importantly, what you would like improved.
